Job summary

Franciscan Care Center in Sylvania, OH is seeking a Licensed Social Worker to support residents and their families through life transitions with care, respect, and empathy.

You’ll assess needs, develop care plans, provide emotional support, coordinate with the care team, assist admissions and discharges, and maintain confidential records. Ohio LSW license and SW degree preferred; experience with older adults is welcomed.

Qualifications

• Bachelor’s degree in Social Work preferred.
• Active LSW license in Ohio.
• Experience with older adults preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Help assess resident needs and develop individualized care plans.
