Job summary

Fundamental seeks a Licensed Social Worker (LSW) to oversee planning, development, and directing the Social Services Department at our Georgetown facility in accordance with federal, state, and local standards.

The role collaborates with discharge planners and families, ensuring smooth tours, follow-up procedures, and comprehensive patient care. Must hold an active Social Work license and have 3+ years in a long-term care setting. On-site, full-time position with competitive benefits.

Qualifications

• Must have an active Social Work license in good standing with the state.
• Bachelor's Degree in Social Work strongly preferred.
• Must have at least 3+ years of experience in a long term care facility.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards.
• Develop and manage the initial contact with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident
• Develop and maintain a good working rapport with intra-department personnel and other departments to ensure social service programs meet patient needs
• Keep abreast of current federal and state regulations and professional standards

The Licensed Social Worker LSW oversees the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards and works directly with discharge planners, family or responsible party to initiate tour of facility and follow up procedures.

Posted Salary Range

USD $30.00 - USD $35.00 /Hr.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

Duties & Responsibilities

• Develop and manage the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents
• Keep abreast of current federal and state regulations as well as professional standards

Qualifications & Requirements

• Must have a current Social Work license, in good standing with the state (as required)
• Bachelor's Degree in Social Work strongly preferred
• Must have at least 3+ years of experience in a long term care facility
• Must be able to organize and prioritize many tasks effectively
• Ideal candidate must be a skilled communicator, director and motivator
• Reliable transportation required
