Job summary

BETHESDA HOME HEALTH AND HOSPICE in Willmar, MN seeks a Licensed Social Worker to provide psychosocial support to clients and families, coordinate admissions, and oversee referrals and quality assurance within the care team.

You will deliver counseling, education, and navigation of community resources, manage the CRM platform, and ensure compassionate, high-quality care for patients with chronic, acute, or terminal illnesses.

Qualifications

• A bachelor’s degree in a human services field including, but not limited to sociology, gerontology, social work, human services, healthcare administration, and psychology.
• Must be licensed by the state of Minnesota.
• Previous social work or gerontology experience is desirable.
• Excellent oral and written communication skills.
• Ability to adapt to differing customer demands.
• General sales and assessment skills based on consumer needs and satisfaction.
• Knowledge of social work practice.
• Knowledge of the health care delivery system and community/state services available for referral.

Responsibilities

• Deliver professional and thorough social work services, including psychosocial assessment and intervention planning.
• Identify patients who require social work assessment and intervention through high-risk screening, interdisciplinary team meetings and individual referrals.
• Conducts assessments and completed individualized plans of care.
• Provides support and counseling to clients and families.
• Provide information and assistance for identified financial or social needs.
• Communicate with potential referrals and oversee the referral process with Admissions Coordinator.
• Provide information to prospective clients and family members.
• Manage the organization’s CRM platform.
• Promote Home Health and Hospice services.
• Coordinate with community director to plan and schedule community events to promote home care services and generate referrals.
• Serve as the client and family liaison and manage grievances.
• Assist with development, implementation, review and revising of a Quality Assurance/Improvement Program.
