Job summary

UnitedHealth Group seeks a Medical Social Worker to provide psychosocial support, assessment and care coordination for patients receiving services in the home. You will collaborate with the interdisciplinary team to connect patients with resources, provide counseling, and promote optimal outcomes in the home setting.

Requirements include MSW/DSW and active LMSW/LCSW licensure, a valid driver’s license, CPR certification, and at least one year of health care social work experience.

Qualifications

• Masters and/or Doctorate in social work from an accredited program.
• Active licensure LMSW or LCSW.
• Driver's License and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide psychosocial support, assessment and care coordination.
• Collaborate with interdisciplinary team to connect patients with community resources.
• Educate patients and families on social work interventions and treatment plans.
