Arden Home Health & Hospice in Brookhaven, MS seeks a compassionate Licensed Social Worker to join our home health team. You will provide psychosocial assessments, counseling, and caregiver support to patients and families in in-home settings.

You will collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans, connect patients with community resources, and ensure compliance with home health standards and regulatory requirements.

We are seeking a compassionate and dedicated Licensed Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and home health care, with experience supporting patients in an in-home or community-based setting.
