Job summary

Qualifications

Responsibilities

• Complete initial eligibility evaluations and required assessments.
• Prepare IFSPs, 6-month progress reviews, annual and exit evaluations.
• Write Evaluation Summary Reports with contributing information for IFSPs.
• Provide direct services using the Family Training Model to support child development.
• Involve parents/families in techniques and activities to promote development.
• Facilitate workshops when appropriate and participate in all required meetings.
• Complete all reports, session notes and documentation; join staff meetings.
