Qualifications

• Bachelor’s degree in social work, gerontology, or a related health and human services field.
• Must be Licensed Social Worker (LSW).
• Minimum of one year of supervised social work experience in a healthcare setting.
• Interpersonal and communication skills with empathy, respect, patience, and discretion.

Responsibilities

• Assess residents’ psychosocial needs and complete social histories.
• Complete assigned MDS sections accurately and on time.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support residents and families with adjustment, rights, and orientation.
• Assist with discharge planning and care transitions.
• Coordinate referrals and act as a liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
• Plan and support resident, family, and community engagement activities.
• Promote facility programs and services to referral partners.
