The ideal candidate holds a Massachusetts Social Worker license and has prior experience in skilled nursing or related healthcare settings, delivering compassionate, person-centered care.

Qualifikationen

• Bachelor’s degree in Social Work, Social Services, or a related discipline.
• Massachusetts Social Worker license required.
• Experience in skilled nursing, long-term care, nursing home, or healthcare settings preferred.
• Strong communication and advocacy skills.
• Ability to collaborate with an interdisciplinary healthcare team.
• Excellent documentation and administrative skills.

Aufgaben

• Provide social work services and psychosocial support to residents and families.
• Assess social, emotional, and psychosocial needs and develop interventions.
• Advocate for resident rights, dignity, independence and quality of life.
• Participate in interdisciplinary care planning meetings and conferences.
