Job summary

Fiberhire is actively seeking a Licensed Social Worker (LSW) in Franklin, MA to join our healthcare team. You will provide comprehensive social work services to patients and residents, collaborating with families and an interdisciplinary care team to support emotional well-being and quality of life.

The ideal candidate holds a Massachusetts LSW license with a BSW/MSW and related experience in skilled nursing or long-term care.

Qualifications

• Current and valid Massachusetts LSW license.
• Bachelor’s or Master’s degree in Social Work from an accredited program.
• Experience in skilled nursing, long-term care, or healthcare settings preferred.
• Strong knowledge of social work principles, patient advocacy, and care coordination.
• Excellent documentation and communication skills.

Responsibilities

• Provide social work services to patients and residents according to care plans.
• Conduct psychosocial assessments and identify needs.
• Develop and implement individualized social work interventions.
• Provide emotional support, counseling, and crisis intervention.
• Coordinate discharge planning and transitions of care.
• Advocate for patients’ rights and quality of life.
• Collaborate with nursing, rehabilitation, medical, and other teams.
• Maintain accurate documentation of assessments and interventions.
• Assist with advance directives and psychosocial needs as appropriate.
