Job summary

Fiberhire is seeking a licensed Social Worker (LSW) for Franklin, MA in a full-time, in-person day-shift role.

You will assess residents’ psychosocial needs, advocate for rights and quality of life, and coordinate care with an interdisciplinary team and their families.

Required: Bachelor’s degree in Social Work, MA LSW license, and experience in skilled nursing or healthcare settings.

Qualifications

• Bachelor’s degree in Social Work required.
• Massachusetts LSW license required.
• Experience in a skilled nursing facility, nursing home, long-term care, or healthcare setting required.
• Strong communication, interpersonal, organizational, and problem-solving skills.
• Resident advocacy and family support skills.

Responsibilities

• Provide comprehensive social work services to residents and families.
• Assess psychosocial, emotional, and social needs and develop interventions.
• Advocate for residents’ rights, dignity, independence and quality of life.
• Participate in interdisciplinary care planning and care conferences.
• Assist with community resources, referrals and support services.
• Maintain confidential documentation per regulatory requirements.
• Support behavioral health and psychosocial needs with referrals.
