Qualifications

• Minimum of one (1) year experience in health care.
• Experience in a long term care setting preferred.
• Bachelor's degree required.
• Current Massachusetts licensure: LCSW or LICSW.
• Demonstrates good verbal and written communication, and organization skills.

Responsibilities

• Assess the psychosocial status of patients and communicate findings to the registered nurse and other members of the interdisciplinary group.
• Counsels patient and family/caregivers as needed in relation to stress and coping.
• Provide crisis intervention when necessary.
• Educate interdisciplinary group on patient/family cultural needs and traditions.
• Maintain clinical records and ensure timely synchronization of patient data with the Patient Care Team.
• Assist in preparation of advanced directives and provide information/referral services.
• Serve as liaison between patients, families/caregivers and community agencies.
• Develop and maintain contracts with public/private agencies as resources for patient care.
• Participate in plan of care development with patient and family and attend interdisciplinary meetings.
• Participate in discharge planning when needed.
