Job summary

Kimball Farms Nursing Care Center Lenox, MA seeks a licensed clinical social worker (LCSW/LICSW) to assess the psychosocial status of patients, communicate findings to the care team, and guide interventions within the interdisciplinary group.

You will handle admissions, discharge planning, crisis intervention when needed, advanced directives education, documentation at Point of Service, and liaison with families and community resources to support culturally sensitive, patient-centered long‑term

Qualifications

• Bachelor’s degree required.
• Massachusetts licensure: LCSW or LICSW.
• 1 year of health care experience; long-term care preferred.
• Strong communication and organizational skills.

Responsibilities

• Assess the psychosocial status of patients and communicate findings to the care team.
• Carry out social evaluations, counsel patients and families, and provide crisis intervention when needed.
• Educate interdisciplinary group on cultural needs, traditions, and family dynamics.
• Maintain clinical records and document at Point of Service.
• Educate and assist in advanced directives and referrals to community resources.
• Participate in discharge planning and plan of care with patient and family.
