Responsibilities

• Acts as client advocate to coordinate services or resolve crises.
• Counsels clients in individual and group sessions to support recovery and adjustment.
• Identifies barriers to client progress from interviews and records.
• Monitors and records client progress against treatment goals.
• Organizes support groups and counsels family members to aid recovery.
• Performs clinical assessments of behavioral health needs.
• Provides case management for identified treatment needs.
• Trains direct care staff on behavior modification and treatment plans.
• Refers clients to community resources for housing, finances, and education.
