Job summary

Regal Care at Wakefield seeks a Licensed Social Worker to join our skilled nursing and rehabilitation team. You will collaborate with residents, families, and the interdisciplinary team to address emotional and psychosocial needs and coordinate discharge planning.

Responsibilities include assessments, care planning, counseling, and ensuring residents’ rights while complying with regulations. A BS in Social Work is required; MSW is preferred, with licensure in good standing.

Qualifications

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
• Current NJ Social Work license (LSW or LCSW) required.
• Experience In Long-term Care Or Sub-acute Rehabilitation Setting Preferred.
• Strong interpersonal, communication, and organizational skills.
• Compassionate and resident-centered approach to care.

• Develop and implement individualized care plans in coordination with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.

• Document all assessments, interventions, and care plans accurately and timely.
