Job summary

• Conduct psychosocial assessments for new admissions and ongoing residents.
• Develop and implement individualized care plans in coordination with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and coordination of community resources and services.
• Serve as a liaison between residents, families, and facility staff.
• Document all assessments, interventions, and care plans accurately and timely.
• Advocate for residents’ rights and ensure compliance with state and federal regulations.

Regal Care at Wakafield is currently seeking a Licensed Social Worker. The Social Worker will play a key role in ensuring the emotional and psychosocial well-being of residents.
