Job summary

Regal Care at Wakefield is seeking a Licensed Social Worker to support residents' emotional and psychosocial well-being. You will collaborate with residents, families, and an interdisciplinary team to assess needs, provide counseling, coordinate discharge planning, and enhance quality of life.

Qualifications

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
• Current NJ Social Work license (LSW or LCSW) required.
• Experience in long-term care or sub-acute rehabilitation setting preferred.
• Strong interpersonal, communication, and organizational skills.
• Compassionate and resident-centered approach to care.

Responsibilities

• Conduct psychosocial assessments for new admissions and ongoing residents.
• Develop and implement individualized care plans in coordination with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and coordination of community resources and services.
• Serve as a liaison between residents, families, and facility staff.
• Document all assessments, interventions, and care plans accurately and timely.
• Advocate for residents’ rights and ensure compliance with state and federal regulations.
