Job summary

AdviniaCare Newton-Wellesley is seeking a compassionate Director of Social Services to oversee residents' psychosocial needs, admissions, care plans, discharge planning, and family advocacy in our skilled nursing center in Wellesley, MA.

You will ensure adherence to NASW ethics and state/federal regulations, collaborate with multidisciplinary teams, and build strong relationships with residents, families, and healthcare professionals to deliver person-centered care.

Qualifications

• Licensed Social Worker with LSW, LCSW, LICSW, or MSW.
• Strong communication, assessment, and social work techniques.
• Ability to work in a fast‑paced environment.
• Maintain confidentiality and handle sensitive information.

Responsibilities

Director of Social Services

Job Summary

We are seeking a compassionate and experienced Director of Social Services to join our dedicated team at AdviniaCare Newton-Wellesley. As a Director of Social Services, you will provide social services to our residents, including admission process, facility adjustment, psychosocial needs, resident grievances, family grievances, MDS assessments, counseling, care plans, discharge planning, and facilitate discharge procedures. Responsibilities

• Manage the psychosocial, mental, and emotional needs of residents residing in a skilled nursing center
• Develop and/or aid in the access of services
• Provide social services in accordance with the National Association of Social Workers (NASW) Code of Ethics and strict adherence to government regulations
• Work effectively with a variety of disciplines in an individual and team setting
• Possess a working understanding of social systems and the ability to implement pertinent state and federal regulations
• Develop and maintain strong relationships with residents, families, and other healthcare professionals

Requirements
