You will assess patients, provide counseling, and coordinate discharge planning while collaborating with physicians, nurses, and case management to support recovery and continuity of care. The role requires a Bachelor’s in Social Work, a current license in the state, and experience in healthcare settings; Master’s in Social Work is preferred.

Qualifications

• Bachelor’s Degree in Social Work required.
• Current, valid and active Social Worker license in the state of employment.
• Master of Social Work preferred; two years healthcare social work experience preferred.

We are seeking a Social Worker to join our team!

Hospital Details

The Meadows of Central Massachusetts is an 82-bed transitional care facility located alongside Vibra Hospital of Western Massachusetts – Central Campus. The Meadows provides comprehensive skilled nursing and rehabilitation services for individuals in need of specialized medical care before they can return home or to another care setting. Many of our patients have suffered serious injuries or illnesses. As a result, they require day-to-day help while they work toward recovery and regain their independence. Our comprehensive, personalized approach to patient care incorporates therapeutic intervention and medical management. We specialize in providing short-term rehabilitation, long-term care, and respite care services.

Responsibilities

Provides patient assessment, counseling, and discharge planning. Acts as a liaison between the patient/family/hospital and community agencies/resources. Demonstrates expertise in social work, working as a member of an inter-disciplinary team comprising a physician, nurses, and case management. Applies specialized knowledge of human development and behavior and social, economic, and cultural systems in directly assisting patients and families to improve or restore their capacity for social functioning. This includes counseling and use of psychosocial interventions and social psychotherapy. Occasionally plans educational offerings and provides in-services as requested.

Required Skills

• Bachelor’s Degree in Social Work required.
• Current, valid, and active license to practice as a Social Worker in the state of employment required.

Additional Qualifications & Skills

• Master of Social Work preferred.
• Two (2) years of experience as a Social Worker in a healthcare setting preferred.
• Ability to project a professional image.
• Knowledge of regulatory standards and compliance requirements.
• Strong organizational, prioritizing and analytical skills.
• Ability to make independent decisions when circumstances warrant.
• Working knowledge of computer and software applications used in job functions.
