Job summary

The Blossoms at North Little Rock is seeking a qualified social worker to support residents in long-term care. You will assess needs, develop individualized care plans, and coordinate admissions, re-entries and transfers.

Experience in social work is required (1 year minimum); LSW/BSW/MSW preferred or a related bachelor's degree. Benefits include health, dental and vision insurance, retirement plan, and paid time off.

Responsibilities

• Maintaining and improving residents' ability to control daily physical needs and psychosocial well-being.
• Identifying individual social and emotional needs.
• Developing and maintaining individualized care plans.
• Participating in admission, community re-entry or transfer planning.
• Joining interdisciplinary care plan meetings to address social and emotional needs.
• Collaborating with finance to educate residents and maintain required documents.

Duties and Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
