Job summary

Richmond Adult Day Care in Staten Island, NY is actively seeking a Licensed Social Worker to support residents in our Adult Day Care program. The role focuses on assessing social and emotional needs and coordinating care plans to enhance daily living and community reintegration.

The ideal candidate holds an MSW or LSW license, has experience in long-term care or subacute settings, and can collaborate effectively within a multidisciplinary team to ensure holistic resident care.

Qualifications

• Must hold an active LSW or MSW licensure.
• Experience in long-term care and/or subacute settings is preferred.
• Strong communication and interpersonal abilities are essential.

Responsibilities

• Maintain and improve residents' ability to manage daily physical and psychosocial needs.
• Identify individual social and emotional needs of residents.
• Develop and maintain individualized care plans.
• Participate in admissions, re-entry planning, or transfers to/from the facility.
• Attend interdisciplinary care plan meetings to address social and emotional needs.

Richmond Adult Day Care is actively hiring a Licensed Social Worker to work in our Adult Day Care located in Staten Island, NY.

DUTIES:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

Requirements:

• Licensed Social Worker (LSW) or Master of Social Work (MSW).
• Previous Long-Term Care and/or Subacute experience required.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.
