Job summary

Cordova Wellness & Rehabilitation Center in Cordova, TN is seeking a Social Services Director to lead our Social Services Department on-site and coordinate resident care transitions within our care team.

We require a bachelor’s degree in social work (or related field) and at least 1 year of supervised healthcare social work experience, with a commitment to resident confidentiality and comprehensive assessments.

Qualifications

• Bachelor’s degree in social work or another human services field is required.
• Minimum of 1 year of supervised social work experience in healthcare.

Responsibilities

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
• Meet with residents and families to assist with medically related services, grievances, or placement needs.
• Complete assessments and participate in comprehensive care plans.
• Facilitate residents’ safe discharge and coordinate transportation for appointments.
• Maintain confidentiality of protected health information in all communications.
• Perform additional tasks as directed by leadership.

Social Services Director Position Summary

As the Social Services Director at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.
• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.
• Complete assessments and participate in comprehensive care plans.
• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
• Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.

Social Services Director Requirements

• A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation or be a licensed Social Worker
• 1 year of supervised social work experience in healthcare
