The position is full-time with standard hours and is supported by Marquis Health Consulting Services. Bachelor's degree is required; a Master’s degree is strongly preferred, along with 1+ year of healthcare social work experience and strong communication skills.

Qualifications

• Minimum of 1+ years of experience as a Social Worker in a healthcare setting.
• Strong knowledge of regulations governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns and emotional needs with ongoing communication to residents, families, and case managers.
• Build and maintain positive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams.
• Develop, implement, and update individualized psychosocial care plans.
• Participate in care planning meetings and support continuity of care.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with facility policies, regulations, and documentation standards.

Responsibilities

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications

• Must be able to work full‑time hours and maintain a flexible schedule.
• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare‑related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast‑paced, team‑oriented healthcare environment.
