Cordova Wellness & Rehabilitation Center in Cordova, Tennessee, is seeking a Social Services Director to lead the department, ensure high-quality resident care, and coordinate with families.

The role requires at least a bachelor's degree in social work or related field, a licensed social worker status, and a minimum of one year of supervised healthcare social work experience.

Qualificações

• Bachelor’s degree in social work or related human services field is required.
• Must be a licensed Social Worker or eligible for licensure.
• At least 1 year of supervised social work experience in healthcare.

Responsabilidades

• Oversee development, implementation, supervision, and evaluation of Social Services.
• Meet with residents and families re medically related services, grievances, or placement needs.
• Complete assessments and participate in comprehensive care plans.
• Facilitate discharge transitions and arrange transportation for appointments.
• Maintain confidentiality of protected health information (PHI).
• Perform other tasks as directed by Administrators or the team.

Social Services Director Position Summary

As the Social Services Director at Cordova Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department.




• Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care.




• Complete assessments and participate in comprehensive care plans.




• Facilitate residents’ safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events.




• Maintain confidentiality of protected health information, including verbal, written and electronic communications.




• Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.

Requirements

• A minimum of a bachelor’s degree in social work or another human services field such as sociology, gerontology, special education, rehabilitation counseling, or rehabilitation or be a licensed Social Worker




• 1 year of supervised social work experience in healthcare
