Job summary

Hc is seeking an experienced Director of Social Services for a Skilled Nursing Facility. The role leads the Social Services Department, champions resident advocacy, and ensures exceptional psychosocial support throughout care transitions.

The ideal candidate will oversee psychosocial assessments, discharge planning, regulatory compliance, and resident experience, with strong leadership and collaboration across disciplines.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or related field required.
• Master's degree in Social Work preferred.
• Current state licensure or certification, if required by state regulations.
• Minimum of two years of social services experience in a Skilled Nursing Facility or healthcare setting preferred.
• Previous supervisory or management experience preferred.
• Strong knowledge of discharge planning, care coordination, and long-term care regulations.

Responsibilities

• Direct and oversee all functions of the Social Services Department.
• Complete psychosocial assessments and develop individualized care plans.
• Coordinate discharge planning and facilitate safe transitions of care.
• Provide counseling and emotional support to residents and families.
• Assist residents and families with advance directives and community resources.
• Participate in interdisciplinary care plan meetings and quality improvement initiatives.
• Advocate for resident rights and promote resident satisfaction.
• Ensure compliance with federal, state, and local regulations governing social services in long-term care.
• Maintain documentation and required assessments.
• Investigate and address resident concerns, grievances, and family issues.
• Collaborate with nursing, rehabilitation, admissions, and other departments to support resident-centered care.
• Prepare for and participate in state and federal surveys.
