Job summary

CareCore Health in Chillicothe, OH seeks a Licensed Social Worker (LSW) to oversee care planning and address residents' social and emotional needs with compassion and professionalism. The LSW will bridge residents, families, facility staff, and community resources to support holistic well-being.
