Continuum Therapy Partners, a SNF rehabilitation provider, is seeking a licensed Physical Therapist Assistant (PTA) for a full-time role serving Marshfield, Stevens Point, and Wisconsin Rapids. Join a collaborative team of PTs, OTs, and SLPs to deliver high-quality rehabilitation for geriatric and post-acute patients, focusing on mobility, strength, balance, and independence in a skilled nursing setting.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Wisconsin PTA license.
• New graduates encouraged to apply.
• Skilled nursing experience preferred but not required.

Responsibilities

• Provide physical therapy treatments under the direction of the Physical Therapist.
• Document patient progress.
• Collaborate with nursing and interdisciplinary teams.
• Assist patients in improving mobility, strength, and independence.
• Follow physician orders and facility policies.
