Job summary

Relient Health is seeking a Licensed Physical Therapist Assistant (PTA) for a Skilled Nursing Facility in Black River Falls, WI. This full-time, direct-hire position offers a supportive rehab team and opportunities for professional growth.

You will provide therapy services under a licensed Physical Therapist, develop and implement treatment plans, document patient progress in EMR, and collaborate with PTs, OTs, SLPs, and nursing staff to enhance patient independence and mobility.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current WI license (or eligibility to obtain).
• New grads welcome.
• Strong communication, documentation, and interpersonal skills.

Responsibilities

• Provide therapy services under the direction of a licensed Physical Therapist.
• Assist in developing and implementing individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs, and nursing staff.
