Relient Health seeks a dedicated Physical Therapist Assistant (PTA) for a Skilled Nursing Facility in Hayward, CA. This full-time/direct-hire role offers day shifts, supportive teamwork and opportunities to grow in a collaborative rehab department.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current CA license or eligibility to obtain licensure.

• Strong communication, documentation and interpersonal skills.

Responsibilities

• Provide skilled therapy services under the direction of a licensed Physical Therapist.
• Assist in developing and implementing individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs and nursing staff.
• Promote patient independence and mobility.
