Job summary

Relient Health is seeking a Licensed Physical Therapist Assistant (PTA) to join a Skilled Nursing Facility in Oakland, CA. You will provide therapy under a licensed PT, help develop and implement treatment plans, and document patient progress.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current CA license (or eligibility to obtain).

Responsibilities

• Provide therapy services under the direction of a licensed Physical Therapist.
• Assist in developing and implementing individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs, and nursing staff.
• Promote patient independence and mobility.
