Relient Health is seeking a full-time Licensed Physical Therapist Assistant (PTA) to join a Skilled Nursing Facility (SNF) in West Reading, PA. The PTA will collaborate with a multidisciplinary team to deliver rehab services and monitor patient progress.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current PA license or eligibility to obtain.

• Excellent communication and documentation skills.

Responsibilities

• Provide therapy services under the direction of a licensed Physical Therapist.
• Develop and implement individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs, and nursing staff.
• Promote patient independence and mobility.
