Job summary

Continuum Rehab Group in Texas is seeking a Licensed Physical Therapy Assistant to join a dynamic team delivering in-house therapy across Skilled Nursing facilities. You will implement evidence-based interventions to support patient recovery and maintain compliance with regulatory standards.

You’ll work under the supervision of a Physical Therapist, assess progress, communicate recommendations, and contribute to discharge planning while ensuring patient and family education and engagement. AA/EEO

Qualifications

• Licensed PTA in good standing
• Open to therapists at all experience levels
• Experience with rehab interventions preferred

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients requiring rehabilitation services.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides consultation and counseling to patients and families.
• Contributes to discharge summaries of services provided as required.
