Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Ft. Worth, TX. This position emphasizes compassionate care, professional growth, and a supportive team culture.

You will provide rehabilitative, medically necessary treatments under supervision, use evidence-based practice, document progress, and assist with discharge summaries while helping residents improve function and quality of life.

Qualifications

• Licensed Physical Therapy Assistant.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions under the supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides counseling to patients and families and other service providers.
• Contributes discharge summaries of services provided per regulatory and clinical requirements.
