Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to join our in‑house therapy team serving Skilled Nursing/Long Term Care Facilities in Independence, MO. You will provide skilled therapy under the supervision of a Physical Therapist and help enhance residents’ recovery and quality of life.

The role emphasizes evidence‑based practice, patient education, and collaboration with the supervising PT to adjust treatments.

Qualifications

• Licensed Physical Therapy Assistant eligible to practice in the state.
• Open to therapists at all experience levels.
• Experience in Skilled Nursing/Long Term Care settings preferred.

Responsibilities

• Uses evidence‑based practice to guide clinical interventions.
• Delivers skilled therapy services under supervision of a Physical Therapist.
• Documents patient progress and communicates changes to supervising PT.
• Provides patient and family counseling on physical disorders and rehab plans.
• Contributes to discharge planning and care coordination per clinical requirements.
