Job summary

Senior Care Therapy is seeking Licensed Professional Counselors (LPC) to provide in-person psychotherapy services to geriatric residents in skilled nursing facilities in NJ, NY, PA, and MD. The role handles administrative duties, allowing clinicians to focus on clinical care, with support from a psychologist-led training and ongoing mentorship.

We offer customizable full-time opportunities, 32- or 40-hour weeks, a sign-on bonus up to $6,000, comprehensive benefits, and a supportive team

Qualifications

• Active and unrestricted LPC license in the state applying for.
• Effective oral and written English communication.
• Proficiency with electronic health records (EHR).
• Strong organizational and documentation skills.
• Ability to provide in-person services at facilities, up to 45 minutes.
• Vaccination and booster status may be required in some facilities.

Responsibilities

• Conduct individual and group psychotherapy sessions in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral health conditions including depression, anxiety, and cognitive decline.
• Develop and implement evidence-based treatment plans tailored to each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members.
• Maintain accurate clinical documentation per regulatory standards using the EHR.
• Monitor patient progress and adjust treatment plans as needed.

Key Responsibilities:

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
• Develop and implement evidence-based treatment plans tailored to the needs of each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
• Monitor patient progress and adjust treatment plans as needed.

Requirements:

• Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
• Effective oral and written communication in English
• Basic proficiency with technology, including electronic health records (EHR).
• Strong organizational and documentation skills, with attention to regulatory compliance.
• Ability to provide in-person services at assigned facilities, up to 45 minutes.
• Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
• Full Vaccination and Booster Status may be required in some facilities.

Preferred Qualifications:

• Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
• History of treating Anxiety, Depression and Adjustment Disorders.
• Psych or Addictions experience is a plus.
• Excellent time management and organizational skills.
