The position requires an active LPN/LVN license, 3 years nursing experience (1 in home health/medical-surgical), strong communication, and CPR certification.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience with one year in community/home health or medical/surgical care.
• Currently licensed as an LPN/LVN in the state.
• Complies with accepted professional standards and practice.
• Strong verbal and written communication and organizational skills.
• Must be a licensed driver with insured vehicle as required.
• Maintains CPR certification.

Responsibilities

• Provides direct patient care under the supervision of a registered nurse.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation per the plan of care.
• Educates patient and family/caregiver about diagnosis and plan of care.
• Coordinates home health services and communicates needs to RN/Clinical Director.
• Uses equipment and supplies effectively and safely.
