• Must have a valid driver's license and insured vehicle.
• At least three years of nursing experience with home health or medical/surgical exposure preferred.
• Complies with professional standards and practice.

Responsibilities

• Provides direct patient care under the supervision of a registered nurse.
• Implements the plan of care and documents accurately and timely.
• Educates patients and families about diagnosis and plan of care.
• Coordinates home health services with other disciplines and reports needs.
• Communicates with the office team and informs of scheduling or patient concerns.
