Job summary

VitalCaring Group is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for home health in Texas. You will provide direct patient care in patients’ homes under RN supervision, delivering skilled nursing services and reinforcing education to support positive outcomes.

Applicants should hold an active LPN/LVN license, a current CPR certificate, and have reliable transportation to cover an assigned territory.

Qualifications

Active LPN/LVN license in the state of practice.
Graduate of an accredited practical/vocational nursing program.
Current CPR certification.
Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
Strong clinical and observational skills.
Ability to follow care plans while reporting changes in condition.
Valid driver’s license and reliable transportation.
Proficiency with EMR systems.

Responsibilities

Provide direct patient care under the supervision of the RN in accordance with the plan of care.
Deliver skilled nursing services, including treatments, medication support, and wound care.
Monitor patient condition and report changes to the RN and care team.
Educate patients and caregivers on self-care, treatments, and disease management.
Maintain infection control and proper use of equipment and supplies.
Document patient care accurately and timely within the EMR.
Communicate effectively with physicians, nurses, and interdisciplinary team members.
Participate in case conferences, team meetings, and ongoing training.
