Qualifications

• Active and unrestricted LPN license in Virginia.
• Experience in home care or geriatrics preferred.
• Reliable transportation and ability to travel to patient homes in Virginia Beach area.
• Excellent communication and interpersonal skills.
• Ability to work independently with minimal supervision.
• Current CPR/BLS certification preferred.

Responsibilities

• Provide direct patient care to senior clients in a home care setting under supervision of a RN or physician.
• Administer medications, wound care, and treatments as directed by the care plan.
• Monitor and document patient health status, vital signs, and changes in condition.
• Communicate effectively with patients, families, and the healthcare team regarding care updates.
• Assist with ADLs and support patient independence and dignity.
• Maintain accurate and timely clinical documentation in compliance with state and agency standards.
• Follow HIPAA, infection control, and homecare safety protocols.

• Monitor and document patient health status, vital signs, and changes in condition
• Communicate effectively with patients, families, and the healthcare team regarding care updates
• Assist with ADLs (activities of daily living) and support patient independence and dignity
• Maintain accurate and timely clinical documentation in compliance with state and agency standards
• Follow all HIPAA, infection control, and homecare safety protocols

Requirements:

• Active and unrestricted LPN license in the state of Virginia
• Previous experience in home care, geriatrics, or a related healthcare setting preferredStrong clinical assessment and critical thinking skills
• Reliable transportation and ability to travel to patient homes in the Virginia Beach area
• Excellent communication and interpersonal skills with a patient-first attitude
