Job summary

VHS Home Health Care is seeking a Licensed Practical Nurse (LPN) to join our team across multiple Virginia locations, including Newport News. We offer full-time, part-time, and PRN opportunities with daily pay.

The role involves delivering high-quality, person-centered nursing care in residents' homes and collaborating with a multidisciplinary team. Qualified candidates hold an active Virginia LPN license, 1–2 years of home health experience, and BLS certification.

Qualifications

• Current, active LPN license in Virginia
• 1–2 years of home health experience preferred
• Valid BLS/CPR certification required
• Ability to manage multiple patients in a fast-paced setting
• Strong clinical judgment and communication skills

Responsibilities

• Provide patient care per standard nursing practices and Plan of Care
• Follow individualized care plans from Nurse Manager and team
• Administer medications and treatments per POC and facility policy
• Monitor residents, report changes to supervising nurse/physician
• Maintain timely documentation per orders and care plans
• Support a safe, respectful environment for residents and families

$30-$34/hour (all inclusive)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Virginia.
• 1-2 years of previous home health experience required.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
