Concordia Visiting Nurses in Western Pennsylvania is seeking a dedicated Licensed Practical Nurse to join our home health team. We provide Medicare-certified, state licensed, Medicaid approved services with a focus on compassionate, dignified care.

As an LPN, you will perform skilled nursing tasks, educate patients and families, maintain records, and collaborate with a broader health care team while enjoying autonomy in scheduling and patient care decisions.

Qualifications

• Minimum of one year of LPN experience (Hospice experience preferred but not required).
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.

What you’ll do as a Licensed Practical Nurse LPN:

As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Assisting with wound care and personal care

• Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.
