Concordia Visiting Nurses in Western Pennsylvania seeks a compassionate LPN to join our home health team. You will provide skilled nursing care, support patients and families, and collaborate with a multidisciplinary group to deliver high‑quality care.

What you’ll do as a Licensed Practical Nurse LPN:

As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records
• Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.

Who you are as a Licensed Practical Nurse LPN:

• A minimum of one year of LPN experience (Hospice experience is preferred but not required).
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and as part of a collaborative team.
• Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.
