Concordia Visiting Nurses Baden in Baden, PA is seeking a dedicated Licensed Practical Nurse to join our home health team. You will provide skilled nursing care, educate patients and families, and maintain medical records while collaborating with healthcare professionals.

As a Home Health LPN, a vital part of the patient\'s care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:
- Assisting with wound care and personal care
- Educating patients and families
- Maintaining medical records
- Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.
- A minimum of one year of LPN experience (Hospice experience is preferred but not required).
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a collaborative team.
- Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.
