Summary
The Licensed Practical Nurse - Primary Care PACT functions in the Ramsey Outpatient Clinic - serving veterans within the Patient Aligned Care Team. This Licensed Practical Nurse provides care and counseling of patients and promotes maintenance of healthy living in the prevention of illness, injury, and chronic disease by using the nursing process, including systematic data collection, monitoring, nursing judgment, and evaluation of human response to actual or potential health problems.

Applicants pending the completion of educational or certification/licensure requirements may be tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- English Language Proficiency
- In accordance with 38 U.S.C. 7402(d) and 7407(d), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.

- Licensure: Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (e.g., Puerto Rico) of the United States or the District of Columbia.
- Grade Determinations: The candidate's qualifications must demonstrate the level of competence required for the grade.
Grade Levels:
- GS-03: None beyond basic requirements. Expected to perform all duties within the scope of their license.
- GS-04: Six months of qualifying experience as an LPN or LVN, or graduation from an approved school with at least 24 months duration, or a year of nursing care work experience.
- GS-05: Completion of at least 1 year of experience at GS-4 level, with demonstrated knowledge and ability to provide full range of practical nursing care.
- GS-06: One additional year of experience at GS-5 level, proficient in initiating, performing, and completing assigned duties, with broad knowledge of practical nursing procedures.
Additional qualifications and skills include:
- Preparation, administration, and documentation of medications and treatments.
- Recognition and response to emergency situations, including resuscitation procedures.
- Ability to recognize equipment malfunctions, safety hazards, and supply deficiencies.
- Ability to serve as a preceptor and actively seek educational opportunities.
Duties
- Maintains clinic flow, performs nursing care such as oxygen therapy, immunizations, medication administration, blood glucose testing, EKG, etc.
- Adheres to standards of practice and safety techniques.
- Collaborates with healthcare team members regarding patient needs.
- Assists with minor procedures, provides patient and family education, and recognizes emergency situations.
- Performs other duties as assigned.
