Job description

Summary

The Licensed Practical Nurse - Primary Care PACT functions in the Ramsey Outpatient Clinic, serving veterans within the Patient Aligned Care Team. This role involves providing care and counseling to patients, promoting healthy living, and preventing illness, injury, and chronic disease through systematic data collection, monitoring, nursing judgment, and evaluation of human responses to health problems.

Qualifications

Applicants pending completion of educational or certification/licensure requirements may be tentatively selected but cannot be hired until all requirements are met.

Basic Requirements:

  • U.S. Citizenship: Non-citizens may be appointed only when qualified citizens are unavailable, per VA Policy.
  • English Language Proficiency
  • In accordance with 38 U.S.C. 7402(d) and 7407(d), proficiency in basic written and spoken English is required for direct patient care positions.
  • Education: Refer to the Education Section of the announcement.
  • Licensure: Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a U.S. State, Territory, Commonwealth, or the District of Columbia.

Grade Determinations:

Qualifications must demonstrate the competence required for each grade:

  • GS-03: Basic requirements only. Performs duties within license scope, with routine or detailed review as needed.
  • GS-04: Six months of qualifying experience or graduation from an approved school with at least 24 months duration, plus relevant experience or training.
  • GS-05: One year of experience at GS-4 level or equivalent, capable of providing full practical nursing care, training less experienced staff, and educating patients/families.
  • GS-06: One additional year of experience at GS-5 level, proficient in patient care, medication administration, emergency response, and supporting complex procedures.

Duties
  • Maintain clinic flow by completing intake procedures, recording vital signs, and measurements.
  • Perform nursing care tasks including oxygen therapy, immunizations, medication administration, blood glucose testing, EKG, insulin pump management, Zio Patch, and PVR bladder scans.
  • Adhere to safe practice standards in medication handling and administration.
  • Collaborate with healthcare team members to meet patient care needs.
  • Assist with minor procedures, ensuring patient comfort and privacy.
  • Provide health education to patients and families about disease management, prevention, and wellness.
  • Recognize and respond to emergency situations, taking appropriate action.
  • Perform other duties as assigned.

Work Schedule: Monday-Friday, 0800-1630. Compensation includes a competitive salary, paid time off, parental leave, childcare subsidy, retirement benefits, and insurance options. Telework is not available. Physical requirements will be evaluated pre-employment.
