Qualifications

• Must have a valid Pennsylvania LPN license or multistate license.
• Experience with G-tube, trach, and vents preferred or willing to train.
• Valid driver’s license and recent PPD or chest X-ray required.

Responsibilities

• Providing one-to-one nursing care in a home environment.
• Following the plan of care set by clinical teams.
• Documenting care provided and any changes to patient condition.

• Taking direction from clinical team and being overseen by a registered nurse
• Following the plan of care

• Valid Pennsylvania LPN License or Multistate License
