Presbyterian Communities of South Carolina is actively seeking a Licensed Practical Nurse (LPN) to join our team at Lexington, SC. You will provide compassionate care, administer medications, and work with residents and families to ensure high-quality healthcare in a homelike setting.

The role requires SC LPN licensure, CPR certification, and 1–2 years of healthcare experience. Weekend and holiday availability is expected; competitive pay and comprehensive benefits accompany this important work.

Responsibilities

• Provide compassionate, individualized care according to residents' care plans.
• Perform regular rounds and monitor residents’ conditions throughout the shift.
• Supervise and direct other care staff.
• Communicate effectively with residents and families about care updates.
• Administer medications and treatments per physician orders.
• Complete documentation accurately and in compliance with regulations.
• Notify physicians and families of significant resident condition changes.
• Assist with resident admissions, discharges, and transfers.
• Maintain clinical competency and uphold professional standards.
• Able to work weekends, holidays, and according to resident needs.
