Job summary

Presbyterian Communities of South Carolina is seeking a dedicated Licensed Practical Nurse (LPN) to support our residents in a warm, homelike setting at The Columbia Presbyterian Community. You’ll work as part of a compassionate care team, delivering person-centered services per individualized care plans.

In this full-time role, you’ll provide rounds, administer medications, supervise staff, document care, and communicate with families.

Qualifications

• LPN license in South Carolina required.
• 1–2 years of healthcare experience preferred.
• Current CPR certification.
• Basic computer skills required.

Responsibilities

• Provide compassionate, individualized care according to residents' care plans
• Perform regular rounds and monitor residents’ conditions throughout the shift
• Supervise and direct other care staff
• Communicate effectively with residents and families about care updates
• Administer medications and treatments per physician orders
• Complete documentation accurately and in compliance with regulations
• Notify physicians and families of significant resident condition changes
• Assist with resident admissions, discharges, and transfers
• Maintain clinical competency and uphold professional standards
• Able to work weekends, holidays, and according to resident needs
