Job summary

Simpson is seeking a Licensed Practical Nurse (LPN) to join our leadership team in a senior care setting. The role focuses on building relationships with residents and their families while providing clinical care, medication administration, and care-plan updates.

The candidate should hold a PA LPN license with at least one year of related experience. On-site duties include supervising care staff, documenting in EMR, and participating in care-termination meetings.

Qualifications

• Graduate of an approved Licensed Practical/Vocational Nursing program.
• Pa LPN License.
• A minimum of one year experience in assisted living, home health or long-term care preferred.

Responsibilities

• Develop relationships with residents and families using a consistent caregiver approach; place resident first.
• Assess, observe, and evaluate resident clinical condition and document findings in the EMR; notify provider/Administrator when necessary.
• Update or modify care plans to reflect changes in care and service needs.
• Supervise and lead care associates.
• Administer medications timely and accurately according to orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents, families, or responsible parties promptly and professionally.
