Simpson House - Philadelphia, PA 19131 is seeking a Licensed Practical Nurse (LPN) to join our leadership team on an evening, part-time basis. The role emphasizes compassionate senior care, medication administration, and care coordination in a supportive environment.

The ideal candidate will have a formal LPN education, PA license, and at least one year of related experience in assisted living or long-term care. EMR experience is preferred to document resident care efficiently.

Qualifications

• Graduate of an approved Licensed Practical /Vocational Nursing program.
• Minimum of one year experience in assisted living, home health or long-term care preferred.
• PA LPN License required.

Responsibilities

• Develop relationships with residents and families using a consistent caregiver approach.
• Assess, observe, and document residents' clinical condition in EMR.
• Update care plans to reflect changes in resident needs.
• Supervise and lead care associates.
• Administer medications timely and accurately.
• Participate in meetings related to resident outcomes and care.
• Respond to residents and families promptly and professionally.
• Review clinical indicators such as falls, wounds, nutrition, and antipsychotic use.

How you will make an impact:

• Developing relationships with the residents and their families using a consistent caregiver approach; supporting the resident’s physical, spiritual, emotional and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in resident electronic medical record and notify the provider and/or Administrator when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates.
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care.
• Responding to residents, family, or responsible party in a timely and professional manner.
• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by Director of Health & Wellness/Administrator

Requirements:

• Graduate of an approved Licensed Practical /Vocational Nursing program.
• A minimum of one year experience in assisted living, home health or long-term care preferred.
• A passion to care for seniors and a strong resident advocate.
• Experience with electronic medical records preferred.
• Pa LPN License
