Plan A Health is seeking a Licensed Practical Nurse (LPN) to provide patient-centered care in home settings for Pediatric and Adult patients with G-tube, Trach, and Ventilator needs. This role involves overnight shifts and requires Virginia licensure, TB testing or chest X-ray, and BLS certification.

You will assess, plan, implement, and evaluate care using the nursing process, delivering services per orders.

Qualifications

• Licensed LPN/LVN in Virginia with current license.
• TB test or Chest X-Ray completed and valid.
• BLS certification required or current.
